HEALTH DEPARTMENT INFORMATION

Every artist tattooing at the Memphis Tattoo Festival must fill out this information. You will need to upload a copy of your Government ID & current Tattoo Permit issued by your local or state government. You should bring a printed copy of your BBP with you to the event. Bloodborne Pathogens Certificate needs to be valid for April 4-6, 2025. If you need a new BBP Certificate, we recommend this one, which is OSHA certified.
